Hodgkin lymphoma survivors' attitudes towards lung cancer screening

Abstract
Introduction: Alkylating agents and thoracic radiotherapy increase
the risk of lung cancer in Hodgkin Lymphoma survivors (HLS) [1].
Lung cancer screening (LCS) has the potential to reduce mortality
through early detection, but has not been established in HLS. Here,
we explore attitudes to LCS as a first step towards developing a pilot
study of LCS in HLS.
Methods: HLS aged 18-80 and ≥5-years since treatment were
identified from a managed follow-up database using random stratified sampling. Thirty semi-structured interviews were
analysed using inductive thematic analysis.
Results: Three themes were identified. Risk perceptions: Most
were unaware of a lung cancer risk, associating smoking and family
history with increased risk and healthy lifestyle with reduced risk.
Former smokers were optimistic about the impact of smoking
on their risk. Prior knowledge or experience of other late effects
mitigated shock and helped process new risk information. Positive
perceptions of LCS: Cancer screening was regarded as an important,
routine intervention, with most willing to participate. Participants
perceived higher cure rates for asymptomatic, early-stage disease;
a view informed by prior experience of early diagnosis. LCS was
viewed as an opportunity to alleviate health-related anxieties
amplified by previous cancer. Overcoming concerns: The desire to
know if one has lung cancer and the perceived benefits of screening
meant few barriers were reported and experience of healthcare was
a facilitator.
Conclusions: HLS feel positively towards LCS and perceived benefits
outweigh concerns. Findings are limited by a lack of current smokers
among participants. Interventions designed to increase awareness
of risk would be a first step in promoting LCS uptake in HLS.
